Schmitts Animal Hospital offers our patient form(s) online so you can complete it in the convenience of your own home or office.

• Fax us your printed and completed form(s) or bring it with you to your appointment.

New Patient Registration Form

We will need your basic patient information on the phone to reserve your appointment in our schedule.  For more complete pet and owner information, please see below form.  If previous medical records are available, please have a copy of your pet's medical record sent or faxed to our office 24 hours before your scheduled appointment for review by our staff. 

Surgical Authorization Form (Required)

This form provides you with important information and options regarding your pet's surgical procedure. We will need this form completed by the morning of surgery.  This helps us tailor our care to meet your needs.  Please read this thoroughly, fill in blanks, including phone number where you can be reached the day of surgery.  If you have questions filling out this form, you can call our office ahead of time (791-2011), or discuss further at check-in on the morning of surgery.

Dental Authorization Form (Required)

Periodontal therapy in pets involves more than scraping tartar from the surfaces of the teeth.  We will anesthetize your pet and this form allows you to make choices for pre-anesthetic bloodwork and IV fluid therapy.  We will need a phone number to reach you during your pets procedure.  If you have questions filling out this form, you can call our office ahead of time (791-2011), or discuss further at check-in on the morning of surgery.

Member Wellness Registration Form - Optional

This form can be filled out to register for access to the member wellness section of our website. You can also sign up for our monthly newsletter to keep up on current health issues and news and events in our office. You can print it out and bring it in to our office or Click Here to register online! The online newsletter sign-up is also on the right.
