STATEMENT OF FINANCIAL POLICY
At Schmitt's Animal Hospital we strive to provide each patient with the best possible veterinary care at a reasonable cost. Prompt payment is necessary to keep our fees to a minimum for all our clients.
Payment in full is due when services are rendered or upon discharge of patient from the hospital.
Payment may be made by cash, check, Visa, Mastercard, Discover or American Express. We also accept Debit cards.
If your check is returned for insufficient funds, there will be a $20.00 returned check fee added to your account.
Accounts over 30 days will incur a 1.5% monthly service charge. Minimum monthly charge is $3.00.
If there are extenuating circumstances and a balance must be charged, a payment agreement must be filled out completely prior to discharge. Arrangements should be made in advance whenever possible and require the authorization of the doctor or office manager.
We reserve the right to discontinue providing veterinary services for any account 90 days past due for which no prior arrangements have been made.
If an account has reached 90 days past due, it may be referred to an outside credit/collection agency. A $20.00 collection agency fee will be added to your account at this time.
Should your account be referred for collection, you are responsible to pay the cost of collection, including but not limited to attorney fees and other litigation fees.
In the case of hospitalization a deposit may be required at the time of admission, with the balance due at the time of discharge.
